School Health Index

The School Health Index (SHI) is a self-assessment and planning guide that addresses school policies and practices related to physical activity, nutrition, and a tobacco-free lifestyle. Developed by the Centers for Disease Control and Prevention (CDC), Division of Adolescent and School Health, it enables schools to:

  • Identify the strengths and weaknesses of school health promotion policies and practices;
  • Develop an action plan for improving student health; and
  • Coordinate the efforts of teachers, parents, students, and the community in improving school policies, practices, and services.

The SHI starts with a simple, straightforward questionnaire that involves administrators, staff, parents, and students working together to create a healthier school. After assessing school policies and practices, the SHI encourages the school to review the results, reflect on the school’s strengths, and weaknesses, and develop a plan for improvement. This funding will be awarded to support schools to implement the changes recommended in the School Health Improvement Plan.

The SHI is designed for use at the school level. However, with minor adaptations, it could easily be used at the district level, especially if the district has only a few schools and those schools have similar policies and practices.
